Overview

Since 1959, Northwest Community Hospital has been more than a place to come to when you're ill – we're also a trusted member of the community where you live. By cultivating partnerships with community leaders and organizations, developing programs that support health and well-being, and reaching out to those with limited resources, we're able to deliver upon our promise to improve the quality of life for our neighbors. It's a promise we made more than 50 years ago, and we're honored to keep the same commitment today.

Senior Outreach

Northwest Community Hospital cares deeply about the older adults in our community and has created special programs and support services designed especially to meet the needs of this unique population. Read about How NCH Cares for Older Adults

Community Outreach

NCH has identified five important health concerns impacting the health and well-being of the community. To address these concerns, NCH has has developed programs – some in partnership with local organizations – that aim to provide greater access to care for the under-served, as well as to provide services that improve the well-being of the entire community.

Community Health Needs Assessment

Although the requirement of conducting a Community Health Needs Assessment (CHNA) and Implementation Plan are new mandates generated by the Affordable Care Act, NCH has been identifying and addressing unmet community needs for more than 50 years.
