All patients come to the Northwest Community Hospital Physical Rehabilitation Services program through referrals from their physicians. In fact, to schedule an appointment with our physical rehabilitation staff, you will need a physician's order. However, your physician does not need to be affiliated with NCH for you to use our physical therapy, occupational therapy, or speech and language therapy services.

Personal assessment and targeted goals

When you arrive at one of our physical rehabilitation centers, we'll begin with a comprehensive assessment to determine your specific needs. Then we'll work with you, your family and your physician to design a personalized rehabilitation plan.

Our goal is to help you reach the highest level of functioning and independence. In many cases, that will involve specialists from several disciplines, including physical therapy, occupational therapy, and speech and language therapy, and treatment in a variety of settings. If you're an inpatient, your physical rehabilitation program may begin in the hospital and continue on an outpatient basis or through home care.

Every rehabilitation plan is designed around a series of measurable goals, and we monitor your progress closely to help you meet them. Education is a central part of the plan in order to give you and your family or caregiver the knowledge and tools to help you reach your goals.

Cardiopulmonary rehabilitation

If you have a chronic lung disorder, NCH's cardiopulmonary rehabilitation program will help you improve your health and prevent future complications. This is separate from Physical Rehabilitation Services. For more information or to visit the Cardiopulmonary Rehabilitation area at the Wellness Center, please call 847.618.7910.

High-risk neonatal developmental follow-up clinic

• Our follow-up clinic focuses on the developmental progress and care of babies who’ve been in our Level III NICU and have had a difficult start because of premature birth or other physical challenges.
• Clinic hours are on Thursdays at the Hospital. Please call 847.618.7761 for the current schedule. We assess babies for progress in growth and development and, if needed, refer families for additional therapies
• Progress reports are sent to your child's pediatrician
• Staffed by licensed physical therapists, occupational therapists and speech language pathologists
• Staff headed by Dr. Joel Fisher, director of neonatology at NCH and an associate professor at Ann & Robert H. Lurie Children's Hospital of Chicago/Northwestern University
