It's easier than ever to access your medical and mental health records

You can request copies of your records online when it’s most convenient for you, 24 hours a day, seven days a week.

We provide two convenient ways to request copies of your medical or mental health records:

• Online via our electronic request forms
• Fax or mail via our printable request forms

In most cases, your medical or mental health records will be ready for you to pick up within 24 hours of receipt of your request, Monday through Friday, 8:00 am to 7:00 pm.

Requests made on Friday evenings, Saturdays or Sundays will be made available the following Monday afternoon for pick up. Specific fees apply according to your request. For your security and privacy, you will be required to pick up records in person at the Patient Services Center at the Hospital, show your ID and sign for them.
