Introducing New C3 Roles to Boost Care Coordination Process
NCH continues to move forward with Aim for Excellence—a new care coordination model that will improve patient flow throughout the hospital system. This is an important initiative on our Road to the Future—one that will help us better anticipate the demand for services and improve our ability to efficiently move patients through their hospital stay.
The role of the Clinical Care Coordinator—referred to as the C3—is central to this initiative. It is a new role for NCH and responsibilities include making sure each patient has a daily itinerary, coordinating testing and treatment schedules, anticipating barriers for service and surveying overall patient flow. C3 employees will work within "the Hub" and as part of each nursing unit to manage a patient’s stay from check-in through discharge.
In the coming months, NCH will add supplemental roles to ensure adequate support across the network. One of these planned roles is Order Organizer, a position that will coordinate daily procedures and tests by collaborating with nursing units and ancillary departments.
