New BOOST (Better Outcomes by Optimizing Safe Transitions) Initiatives Developed

NCH has been a BOOST hospital since October 2012. The goals of the BOOST Program are to improve patient satisfaction around the hospital discharge process, reduce 30-day readmissions, and mitigate risks for adverse events associated with hospital transitions and readmissions.

Since the initiation of this program sponsored by Blue Cross Blue Shield of Illinois, the Society of Hospital Medicine, Northwestern University and the Illinois Hospital Association, the BOOST Interdisciplinary Team at NCH has developed several key initiatives that benefit patients to transition successfully to their next setting:

  • Implementation of "best-practice" teach-back methodology to ensure patient understanding and compliance with post discharge instructions
  • Simplification of patient discharge instructions format, including a change from handwritten to computer print to ensure clarity and legibility
  • Implementation of an evidence-based readmission risk assessment tool (Transition Risk Assessment) on all admissions on the Medical Pulmonary PILOT Unit, which facilitates:
    • Patient-specific teaching for high-risk medications and diagnoses
    • Pharmacist education of patients at bedside about administration and side effects of high-risk medications
    • Pharmacist formal evaluation of polypharmacy (>10 scheduled medications)
