NCH is implementing Aim for Excellence, a new care coordination model that will improve patient flow and give physicians and hospital staff the ability to more easily and effectively manage resources during a patient's stay. Patients and caregivers will have a daily itinerary that tells them when procedures, diagnostic testing, or potential discharges or transfers will take place.
Aim for Excellence impacts NCH's entire infrastructure for how we manage a patient's experience at the hospital. It focuses on efficiently anticipating, prioritizing, coordinating, scheduling, preparing for and tracking all patient care, services and transport. This means that anyone who has a hand in patient care will be impacted by Aim for Excellence. Nurses, care managers and social workers will experience the greatest change to their roles.
Karen Colby, Vice President Patient Care Services, and Pat Stack, Vice President Performance Improvement, hosted a series of information sessions for staff to discuss the Aim for Excellence initiative. They shared information about the impact on day-to-day operations, staffing and schedules, and available staff training and support. In addition, we have developed a Questions & Answers document to further explain the program and provide background. We have posted the Q&A to the NCH Intranet and the document will also be included on CareLink's My Weblinks.
Aim for Excellence is an important step on our Road to the Future and an important change that will help us improve efficiency, quality and patient care. We will provide staff training and other support as this new care coordination model is developed at NCH, and we encourage you to learn more in the coming months.
