Financial Information
- On your behalf, Northwest Community Hospital will bill your primary and secondary insurance plans if you provided us with this information at the time of registration.
- You are ultimately responsible for payment of all charges if your insurance does not pay.
- If you are not insured, payment in full is expected within 30 days from the billing date.
- If you are unable to pay your bill within 30 days, contact a Customer Service Specialist at 847.618.4747 to set up a payment plan or to discuss other arrangements.
Billing Policy
Please be aware that each time you receive services at Northwest Community Hospital or Day Surgery Center, a separate account is created. If you are receiving monthly ongoing services, a monthly account is created. It is possible to have several accounts open at the same time for which you will receive separate statements. Your statements may reflect accounts billed to your health plan that are not yet resolved, or balances that you owe.

Billing For Separate Services
- Depending on the services provided, you may receive separate bills from other health care professionals who provided services to you at Northwest Community Hospital or Day Surgery Center.
- Charges for anesthesiologists, radiologists, pathologists, cardiologists, surgeons and attending physicians are billed separately.
- These separate services may not be contracted through your HMO or PPO. For your convenience, we are listing several frequently requested numbers:

Paying Your Bill
Northwest Community Hospital accepts:

Payments can be made in person at the cashier's office located near the Visitor Entrance of the hospital. To pay by credit card over the phone, call 847.618.4747.
By mail, please include your patient account number on the front of the check, insert the stub from your statement and send the payment to:

Financial Assistance
At some time, almost every family is touched with an unexpected need for medical care. We accept Illinois Public Aid. In addition, you may qualify for financial assistance through Northwest Community Hospital. If you need help in meeting your hospital bill obligations, please contact our office at 847.618.4747.
