Diabetes Services - Welcome

Understanding diabetes and all of its aspects leads to better diabetes management and control. Because we know this disease impacts more than just the patient, family members are encouraged to join in the educational process.

Diabetes education is conducted during individual appointments in the outpatient clinic for those with Type 1 and Type 2 diabetes, and is facilitated by certified diabetes educators and experts in exercise and mental health. Physician referral to the program is recommended. The American Diabetes Association recognizes Northwest Community Healthcare's diabetes patient education program for Quality Patient Education.
