INDIVIDUAL SERVICES

Outpatient Services Medical Nutrition Therapy
Personalized counseling conducted by a registered dietitian who works with you to establish an appropriate meal plan.

Glucose monitoring
The principles of glucose monitoring and instruction on the use of glucose meters are presented.

Medication Instruction
The proper use of oral medications and insulin is reviewed.

Healthy Living Skills
Patients learn helpful tips for controlling blood sugar can be throughout life.

Emotional Support
A mental health liaison is available for consultation.

Outpatient Services for Women with Gestational Diabetes
Sometimes during pregnancy gestational diabetes can occur. Proper diet and glucose monitoring are important aspects of protecting the health of mother and baby.

Medical Nutrition Therapy
Personalized, one-on-one counseling conducted by a registered dietitian who helps establish meal plans during pregnancy

Glucose Meter Loan Program
A glucose meter, startup supplies and instructions are provided

External Insulin Pump Program
Education and initiation of the external pump is done on an outpatient basis by working with the patient and his/her doctor to help build confidence with therapy. The external insulin pump helps normalize blood sugar and give patients more lifestyle flexibility.

 
 
 

GROUP SERVICES

Diabetes Care Series
Certified diabetes educators present a three-part program each month as an introduction for those recently diagnosed with diabetes, and those who may need a review of diabetes management. Current practices in personal meal planning, medication, glucose monitoring, exercise and day-to-day management skills are discussed.

Diabetes Support for All Ages
A variety of diabetes support services are offered for all ages through Northwest Community Hospital. For times, location and more information call Diabetes Services at 847.618.4475.

Parents and Young Children
2nd Thursday of every month

Parents and 5-10 year olds
2nd Tuesday of every month

Adult Daytime Group
3rd Tuesday of every month

Adult Evening Group
3rd Thursday of every month

Additional Services Diabetes Resource Materials
Videotapes and books are available for loan on such subjects as glucose monitoring, exercise, and foot care. To set up an appointment to review or check out these materials, call Diabetes Services at 847.618.4475.
