When you or your loved one requires care in the home, you want to be sure that the services provided are of the highest quality. In order to enhance your ability to function independently at home, we offer personalized one-on-one care, 24-hour patient access to a registered nurse and regular physician updates.

What Services Does Northwest Community Home Healthcare Provide?

Northwest Community Home Healthcare offers a comprehensive range of services, which include:

Skilled Nursing (to read about our Exceptionally High Customer Satisfaction, click here) which serves as the foundation for all home care services. You will be assigned your own primary nurse. As a patient, you have the advantage of receiving care from nurses with extensive experience in cardiac and pulmonary rehabilitation. There are also many nurses certified in areas of specialty that include:
  • Wound care, ostomy care, use of wound healing equipment
  • Diabetes, including management of diet, medication regime, exercise program, and prevention of complications
  • Intravenous therapy or infusion therapy
  • Psychiatric care, including medication education, symptom management, and counseling
  • Case management to maximize insurance coverage of home care services

Rehabilitation Services (to read about our Exceptionally High Customer Satisfaction, click here) are the key to independent function after a hospitalization or an extended stay at a nursing home. As a patient, you will receive care by therapists with advanced degrees and experience in various methods of evidence-based practice. The types of therapy available are:

  • Physical therapy for gait training, home exercise programs, post-surgical care
  • Occupational therapy for upper extremity exercise programs, and adjustment to challenges of daily living after illness or surgery
  • Home Health Aides for assistance with bathing and personal hygiene, meal preparation, and carrying out an exercise program
  • Speech Therapists for assistance to regain the ability to produce and understand speech as well as facilitate communication skills.

Medical Social Services (to read about our Exceptionally High Customer Satisfaction, click here) are available through Social workers, who have advanced degrees and experience in counseling and community services. You will benefit from their knowledge on ways to cope with the challenges of remaining healthy and independent in your home. Some of their areas of expertise include:

  • Counseling for anxiety, depression, adjustment to illness
  • Pain management and relaxation techniques
  • Advanced Directives, such as living wills and healthcare power of attorney
  • Financial assistance with prescriptions, utility bills, hearing aids, eyeglasses
  • Education on hiring a caregiver to help with errands, household chores

Who is Eligible for Home Care Services?

You are eligible for home care services if you have difficulty leaving your home and require intermittent and skilled health care.

Northwest Community Home Healthcare accepts Medicare, Medicaid and most insurance plans. Financial assistance is available to those who need it.
