We provide one-on-one home care with 24-hour patient access to registered nurses and regular physician updates. Home Care is not only a desirable health care delivery system for many patients; it is also efficient and effective. Skilled nurses serve as the foundation for all home care services

They are certified in areas of specialty including:

  • Wound care, ostomy care and use of wound healing equipment
  • Diabetes diet management, education on medications, exercise and prevention of complications
  • Intravenous and infusion therapy
  • Psychiatric care including medication education, symptoms management and counseling

Modern Technologies

Northwest Community Home Care clinical professionals use modern equipment to evaluate and assess patients. All of our employees are experienced and trained to provide instruction on specialized medical equipment, which sets us apart from other home care agencies. They carry the following equipment with them on home visits to readily provide the best care possible:

  • Cardiac monitors to conduct cardiac rehab exercise in the home
  • Glucose monitors to regulate diabetes
  • Protime monitors to check blood clotting ability and monitor the use of anti-coagulant medications
  • Pulse oximeters to monitor the oxygen saturation levels
  • Ultrasound equipment used for rehabilitation and treatment of pain
  • Vacuum assisted wound devices to aid in wound healing
  • Laptop computers for documentation of the patient’s clinical condition

Social Work

Social workers counsel and educate patients. Working with patients and their families, social workers maximize strength and coping skills, promote patient self-determination and self-advocacy. They have advanced degrees and expertise in holistic approaches to care, including: psychotherapy and relaxation techniques, personality trainer certification, including conflict resolution and interpersonal communication skills, geriatric certification, dementia assessment, care and management certification, and ethics certification.

Rehabilitation Services

Orthopedic, neurological and stroke rehabilitation helps orthopedic and neurological patients reach their maximum level of rehab and prior level of function.

Treatment 2

Wound Care

Experienced nurses work toward healing complicated wounds, pressure ulcers and skin care problems. Relief from pain and infection is emphasized. An extensive array of products are available for faster wound healing.

Treatment 3

Home Infusion Therapy Services

Patients are able to have infusion therapy in the comfort of their home instead of staying lengthy periods of time in the hospital or at an extended care facility. Home infusion requires participation by the patient or a caregiver to receive instruction and manage the infusion in between nursing visits. NCH provides nursing care for a broad range of infusion therapies: pain management, fluids for hydration, total parenteral nutrition, and antibiotics.

Mental Health Care

Most patients are referred through their physicians, however you may request services directly if you have difficulty leaving your home and require intermittent and skilled health care.

Cardiopulmonary Restorative Services

NCH helps ease the transition from hospital to home for patients who have experienced heart attacks, angioplasty, open-heart surgery or pulmonary illness.

Diabetes Education

A certified diabetes nurse educator, a dietitian, and a team of healthcare professionals monitor and educate diabetes patients. NCH Home Care is the only home care agency in the state of Illinois that has met the rigorous criteria to be recognized by the American Diabetes Association (ADA) to administer diabetes education in the home setting.

Therapists serve an important role in the rehabilitation of each patient. They have experience in both traditional and alternative forms of rehabilitation techniques including:

Traditional therapy

  • Neurodevelopmental techniques for neurological disorders such as Parkinson’s disease, strokes, Multiple Sclerosis, and brain injury
  • Orthopedic expertise for shoulder, hip and knee disorders, osteoporosis and arthritis
  • Lymphedema therapy
  • MacKenzie therapy techniques for back injuries
  • Vestibular Rehab techniques for dizziness

Alternative therapy

  • Reiki
  • Cranio-sacral therapy for back pain and injury
  • IMT – Integrated Manual Techniques, such as myofascial release
