Hospice &
Palliative Care

Hospice begins when a terminally ill patient chooses care options focused on comfort and symptom relief rather than on a cure. Quality of life and psychosocial and spiritual support for patients and their loved ones is emphasized.

Hospice care is typically provided within the home, but sometimes more intensive 24-hour care is required. To meet that need, we have partnered with JourneyCare® Hospice Care Centers to offer dignified medical care in a comfortable, home-like environment. Our hospice suites are located on the ninth floor of the East Pavilion.

Inpatient Hospice Care

In certain situations, a patient’s condition might be too severe to care for them at home. Our partners at JourneyCare provide professional, compassionate care while offering up all the comforts of home. Their staff will assist patients, as well as their families, through this time of transition.

How do you find care?

While most patients are referred by their physicians, anyone can initiate hospice care. A JourneyCare representative can come to you to discuss hospice and determine a plan of care that meets your needs. Admission to hospice is based on a physician’s and hospice medical director’s judgment that hospice care is appropriate for an individual. We will work with JourneyCare to coordinate all details related to transitioning to the hospice suite.

In-home Hospice Care

Hospice care provides medical management, emotional support and spiritual resources for patients who are in the last stage of a terminal illness and want to live out the rest of their days at home. It focuses on quality of life instead of treatments to prolong life. Hospice care provides comfort and dignity to patients and helps family members manage the practical details and emotional challenges of caring for a terminally ill loved one.

Palliative Care

Palliative care is for anyone with a serious illness that causes discomfort, anxiety or other physical or emotional distress. This specialized type of care aims to improve your comfort and quality of life by relieving symptoms associated with a serious illness. Palliative care can begin at any stage of a life-altering illness, simultaneously with treatments that have cure or recovery as a goal.

Our palliative care team knows that managing an acute or complex illness can be difficult for you and your loved ones—both physically and emotionally.

That’s why we have partnered with JourneyCare to bring you specialized palliative medicine clinicians who are experienced with coordinating complex medical care focused on your comfort and healing. We’ll help you understand your prognosis and comfort care options while also providing support to loved ones.

NCH was the first hospital in Illinois and among the first 10 acute care hospitals in the nation to receive Advanced Certification for Palliative Care from The Joint Commission, ensuring patients and families are receiving comprehensive and holistic palliative care services that meet specific standards.

With the aim of helping to enhance your medical care and manage your illness, the palliative care team can provide:

  • Advance directives planning
  • Counseling to manage stress and anxiety
  • Emotional support and advocacy
  • Pain and symptom relief
  • Spiritual guidance

Anyone can request a consultation for palliative care, but often your doctor will make the recommendation. If you think palliative care might be right for you or your loved one, please talk about it with your doctor. Or call us at 847-467-7423 to schedule a consultation.
