Community Nursing

This program serves two unique populations who often have difficulty navigating the healthcare system: under-resourced individuals and older adults.

A bilingual community nurse at the Community Resource Center works with patients to assess their immediate needs and collaborates with the community clinics to accommodate urgent cases and provide referrals.

Community nurses also address the unique healthcare needs of older adults through prevention and education. They help address those needs through chronic disease management, medication management, diabetes support groups, healthy cooking demonstrations, nutritional counseling, blood pressure screenings and walk-in office hours. All services are free.
